Referrals deserve a clear, careful handoff.

BloomHart welcomes appropriate referrals for telehealth psychiatric-mental health assessment and ongoing care for people ages 12 and older, subject to clinical fit, location, capacity, and patient choice.

Before referring

Please do not send patient information here.

The public website intentionally has no referral form or unsecure upload. Do not send protected health information, records, identifiers, or urgent clinical information through general website tools or unsecure email.

A secure referral route will be activated only after the practice has completed its privacy, consent, and workflow setup.

What helps us assess fit

Patient basics

Age, current physical location for telehealth, and preferred contact route through a secure workflow.

Reason for referral

A brief clinical question, current supports, and the level of care being sought.

Urgency and safety

Whether the patient is stable for routine outpatient telehealth and has needed local supports.

Patient choice

Patient or guardian awareness and consent to the referral, as applicable.

Urgent needs

Not for emergency referrals.
