Extended evaluation
$425
Up to 75 minutes when additional complexity or coordination is anticipated.
Follow-up visit
$165
Up to 30 minutes for ongoing care and medication management when appropriate.
Extended follow-up
$225
Up to 45 minutes when more time is clinically appropriate.
Care coordination
$165
Up to 30 minutes for clinically appropriate coordination or caregiver consultation.
Rates are per visit and do not guarantee that a particular service is clinically appropriate or available. Fees, appointment length, cancellation terms, and any additional administrative charges must be finalized in the practice's written financial policy before launch.
How self-pay works
What to expect before your first visit.
- Confirm fit and location. We verify age, where the patient will be located during telehealth, service needs, and availability.
- Receive written information. Before care is scheduled, you receive the relevant fee, cancellation policy, and a Good Faith Estimate when required or requested.
- Complete secure intake. Health information, consents, and payment are handled through the approved secure workflow, never a general website form.
- Pay directly to the practice. Payment timing and accepted methods are set out in the final financial policy before your visit.
Insurance and reimbursement
Can I seek reimbursement?
You may ask your insurer whether it offers out-of-network reimbursement. BloomHart does not promise reimbursement, coverage, or payment by any plan. If the practice provides a receipt or superbill, it is for you to submit directly to your insurer.
